Request to Increase Grant-in-Aid

The following form allows you to request an increase in Grant-in-Aid for a student athlete:

Directions on using the form

  1. Open the Request to Increase Grant-in-Aid form by clicking on the above link.
  2. Choose open.
  3. Place your cursor on the "Academic Term" you want and click
  4. Tab to the next section
  5. Complete the form
  6. Print two copies of the form
  7. Coach signs the completed form
  8. Compliance Officer signs completed the form
  9. Athletic Administrator signs the completed form
  10. Give one copy of the signed, completed form to the Athletic Business Manager
  11. Send one copy of the signed, completed form to Gerald Curd in the Financial Aid Office
