Staff Declaration Form

The following form is used to identfy a program's coaching staff initially and identify staff changes that occur during the year. Staff Declaration Form
 Directions on using the form Open the Form used to identfy a program's coaching staff by clicking on the above link. Then choose open.
  1. Fill out the form online
  2. Print the form
  3. Close the Adobe program
After you have a hard copy of the form complete the following steps
  1. Sign the form
  2. Turn the form into the compliance office
  3. Keep the copy for yourself
