Prospect Notification Form and Guidelines

This form will notify the Compliance Office and Academic Advisors of prospective student-athletes. Prospect Notification Guidelines

 Directions on using the form Open the Prospect Notification form by clicking on the above link. Then choose open.
  1. Complete the form
  2. Submit the Prospect Notification Form and high school transcripts and test scores (if applicable) to the Compliance Office
  3. Compliance Office will review the form and complete an evaluation per the guidelines.
