Official Visit Form and Checklist

The following form is used to request an Official Visit for a prospective student-athlete. Official Visit
Student Host Request Form
Directions on using the form Open the Official Visit form by clicking on the above link. Then choose open.

Complete the form
Submit the Official Visit Form and high school transcripts and test scores (if applicable) to the Compliance Office
Compliance Office will review the form, complete an evaluation, and record prospect in Institutional Request List (IRL)
Compliance Office will confirm approval of the Official Visit with the Coach via email

Note:

If there will be a student host, complete the student host paperwork.
If the Prospective Student-Athlete will be needing complimentary tickets for a home competition, complete the Official/Unofficial Pass List paperwork.
