Fee For Lesson

The following form is used to inform student athletes of the requirements involved with compensation for coaching/teaching sport skills in their sport on a fee-for-lesson basis. Fee For Lesson
 Directions on using the form Open the Fee For Lesson form by clicking on the above link. Then choose open.
  1. Fill out the form by typing in the fields
  2. Print the form
  3. Close the Adobe program
After you have a hard copy of the form complete the following steps
  1. Obtain the signature of the Compliance Coordinator
  2. Turn the form into the compliance office
  3. Keep the copy for yourself
