Exit Interview for Non-Returners

The following form evaluates the status of your student athlete. Exit Interview for Non-Returners
 Directions on using the form Open the Exit Interview for Non-Returners form by clicking on the above link. Then choose open.
  1. Place your cursor on the "Name blank space" and click
  2. Type the name of the Student Athlete and tab to the next section
  3. Continue filling in the information on each section until you tab back to the top of the form
  4. Print a copy of the form
  5. Close the Adobe program
After you have the hard copy of the form complete the following steps
  1. Meet with the Student Athlete informing them of this evaluation
  2. Student Athlete signs the form
  3. Coach signs the form
  4. Give the form to the Compliance Officer
