IPFW Athletic Equipment Sheet

The following form is used to issue and collect team equipment for student athletes. IPFW Athletic Equipment Sheet
 Directions on using the form Open the IPFW Athletic Equipment Sheet by clicking on the above link. Then choose open.
  1. Student Athlete completes the form with the equipment manager.
  2. Obtain the signature of the student athlete when equiment is issued and collected.
  3. Equipment Manager keeps the form for future reference.
  4. Equipment Manager will turn the form into the compliance office and the athletic business office if needed.
