Student Athlete Employment Request Form

The following form is used to approve student athlete employment requests. Student Athlete "Fee For Lesson" Employment Request Form
Student Athlete "Camp" Employment Request Form
Student Athlete "All other" Employment Request Form
 Directions on using the form Open the applicable Student Athlete Employment Request form by clicking on the above link. Then choose open.
  1. Fill out the form by typing in the fields
  2. Print the form
  3. Close the Adobe program
After you have a hard copy of the form complete the following steps
  1. Obtain the signatures required to complete the form
  2. Turn the form into the compliance office
  3. Keep the copy for yourself
